CRSI is now hiring LPNs. BASIC FUNCTION: To provide quality nursing services to consumers based on each consumer’s Medical needs and Individual Plan. Ensure that CRSI ICFs operate under the current standards and regulations established by DODD Medicaid (Title XIX), state DODD Licensure, County Quality Assurance, and other authorities that may dictate standards of operation including nursing services, consumer safety, and staff training.
Follow established policies and procedure set by the agency. To include, but not limited to, keeping accurate records, ensure compliance with all trainings and employment requirements, etc. Monitor and ensure the consumer’s safety, health, and welfare. Provide nursing services according to current standards of practice Complete routine assessments in coordination with the Lead Nurse and ICFIID RN Coordinator as required for regulation and consumer need and assist in the preparation for any meetings concerning the consumer to include quarterly and annual reviews. Prepare documentation for Annual Meetings, Physician Visits, Preventive Health Appointments, emergency transfers, and others documentation as assigned Administer medications and treatments as prescribed by the physician according to standards of practice and company procedure Make shift rounds to monitor consumer condition according to assessed needs Complete assessment and, if indicated, treatment of illnesses and injuries Provide emergency care services as needed Maintain communication with staff of any day services to assure continuity of care Communicate with attending physicians, request and receive orders, and transcribe if required Maintain communication with pharmacy to assure prompt delivery of needed medication and treatment supplies Appropriately document all services provided according to standards of practice Maintain a clean and safe environment and be sensitive of the consumer’s and family needs. Participate in an infection control program to include surveillance and outbreak management Always maintain the highest standard of conduct and courtesy in dealing with consumers, family, staff, and the community. Promote positive interactions. Attend and participate in scheduled staff meetings as well as designated trainings and in-service sessions. Monitor storage of medications for ample supply and expiration and place orders to replace Perform other duties as assigned.
Must possess a current License for Practical Nurse or Registered Nurse in the State of Ohio Must have a valid driver’s license and vehicle insurance. Must possess acceptable police record and background screenings. Prefer at least 1 year nursing experience Professional Liability Insurance is recommended
This position will require standing, sitting, walking, bending, lifting of moderate to heavy amounts, pushing and pulling for moderate periods of time. May involve a degree of risk to back injury if proper lifting procedures are not followed. Additional hazards may include injuries related to behavioral incidents during consumer interaction.

The ICFIID has nursing coverage 24 hours a day. The shift start and end times are determined by the location and its unique needs.. The work environment for this position may include risk of exposure to certain communicable diseases if proper hygiene techniques and proper protection apparatus on not utilized.
