Champaign County Department of Job & Family Services is now hiring.
RESPONSIBILITIES:
Primary duties of this position will be to determine eligibility for SNAP, Medicaid, and Title XX public assistance benefits to customers who are determined Aged/Blind/Disabled, living in Long Term Care or receiving Home Community Based Services (HCBS), Special Recovery Services, or Disability Determination Medicaid. The person will be performing case management and customer service type duties daily while maintaining an individualized caseload and a shared case bank.

MINIMUM QUALIFICATIONS:
Must have good verbal and written communication skills. Excellent time management skills and ability to multi- task with a strong attention to detail. A year prior experience as Eligibility Referral Specialist 1, or experience in human services, behavioral or social science, education or related field; OR education or experience in interviewing techniques, business math, case preparation techniques, business writing, customer service techniques and computer technology; OR high school diploma (or equivalent) plus work experience which meets the minimum essential functions of the position.

PREFERRED QUALIFICATIONS:
Knowledge of laws and regulations governing Public Assistance Programming, Ohio Administrative Code (OAC), explain rights, responsibilities, requirements, and regulations. Experienced or have working knowledge of Medicaid Information Technology System (MITS), Ohio Integrated Eligibility System (OB), Hearing and Appeals Tracking System (HATSX) as back up, Ohio Enterprise Document Management System (EDMS), Compass Pilot, and other state supported or internet systems.

Caring Kitchen is looking for a part-time Case Manager to work approximately 25-30 hours per week. The successful candidate will assist residents of Caring Kitchen emergency shelter with access to supportive services. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s service needs.
Responsibilities
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable and client-centered
  • Handle case assignments, draft service plans, review case progress and determine case closure
  • Complete assessments and provide clients with immediate access to safety through emergency shelter
  • Conduct needs assessments and develop individualized case management plans with clients
  • Provide crisis intervention. Assist victims of domestic violence to develop safety and housing plans
  • Collaborate with other social service agencies and law enforcement to ensure equal access to services for all clients
  • Network with other social service agencies and community groups
  • Adhere to professional standards as outlined by shelter policies and procedures
  • Assist with the training of volunteers and interns
  • Record case information, complete accurately all necessary forms and produce statistical reports

POSITION SUMMARY:
This position’s first priority is volunteer recruitment for the Meals-on-Wheels program in Champaign and Logan Counties. This may include recruiting volunteer delivery drivers, monitoring the documentation to meet funder requirements, reporting client and volunteer requests and concerns, providing training sessions, and scheduling visitors and staff to go on routes. Additionally, this position may assist with assessments for active clients. Work requires attention to detail, customer relations, communication skills, interviewing skills, understanding of seniors and handicapped individuals, aging network and funding sources and an ability to handle multiple functions at one time.  This position will have frequent contact with agency personnel, volunteers, clients, funders and case managers and the general public. Work performed will be of a professional and sometimes confidential nature.  A positive and cooperative image and attitude is crucial, as well as independence and self-direction to accomplish its essential functions and responsibilities.
 
PRINCIPLE RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
·         Responsible for assisting with the recruitment of volunteer delivery drivers. 
·         Responsible for assisting with daily coverage of volunteer routes. 
·         Responsible for securing volunteer driver documentation and producing reports. 
·         Responsible for assisting volunteer services staff with providing orientation ride-a-longs for student interns, agency visitors and newly hired agency personnel. 
·         Responsible for assessing clients and eligibility, including paperwork and follow-up.   
·         Performs other departmental and agency functions as necessary, including orientation and training of departmental staff, attending functions, participating in meetings and committees as necessary, route delivery and performing other assignments as required.
 
WORKING CONDITIONS: Work in designated office site, meal sites, and client homes.  Part time or full time opportunity, generally working between the hours of Agency opening and closing, Monday through Friday, some weekend and evening work may be required on a limited basis.
 
QUALIFICATONS: Position requires a minimum 1 year experience in an outreach/recruitment position or equivalent, and strong customer relations, multi-tasking skills and be detailed oriented.
 
KNOWLEDGE, SKILLS, AND ABILITIES:
  • Knowledge of or ability to learn about counties served, including outreach.
·         Ability to communicate with a diverse public, including older adults and handicapped individuals.
·         Knowledge of community-based services and aging network in communities served.

Investigates claims of child abuse/neglect/dependency and reports to law enforcement as indicated in the Champaign County Memorandum of Understand-ing. Interviews alleged victim (s), interviews significant others, and interviews collateral contacts.

Maintains caseload of children/families who have previously been determined to require the protective assistance of the services of the agency.

Determines initial eligibility for intake/referral. Prepares assigned cases for administrative or judicial procedures as required. Determines and re-determines clients’ eligibility for publicly funded social service programs. Maintains con-tact with numerous community and other governmental agencies to coordinate services and obtain information.

Completes reports, case recordings and correspondence. Participates in legal action to establish legal guardianship in cases, to include testifying in court as necessary. Meets procedural deadlines governing the management of social services cases set by the State of Ohio and ensures that this documentation is complete.

Serves as on-call social service worker (e.g., on weekends, holidays, and after business hours). Responds to assigned emergency calls and situations. Answers and returns phone calls from consumers and colleagues.

Attends Social Services related training, conferences and other educational programs as required for position. Other duties as assigned. Serves as on call and back up for Adult Protective Services.
Education and Experience Required

Required:  Bachelor's degree in social work or related social services field.
Preferred: Experience in child protective services and/or CORE training or adoption assessor experience

BASIC FUNCTIONS:


Perform all functions of recruiting and hiring, maintenance of personnel files and keeping all required records current; updating the Human Resources Information System; assist with employee relations issues and performance management, assist with external and internal reviews and audits, and perform other specified projects as assigned.

PRIMARY RESPONSIBILITIES:

Perform all functions of recruiting including communicating with managers to help determine staffing needs, creating and posting internal, assist with interviewing applicants (when needed), and completing a variety of background and employment checks as well as all other pertinent paperwork of new hires including abuser registry, nurse's aid registry, Med Admin, Fraud, and BMV checks, etc.

Conduct new hire orientation for new employees. May be required to become a CPR/FA instructor and assist in training. Keep employment materials updated and current. Prepare materials for new hire training, annual training, etc.Ensure accurate data entry into the HRIS. Maintain personnel files and HRIS information keeping them current and updated.

Communicate monthly with managers to assist in keeping employee requirements and training up to date.

Generate various reports as requested.

Assist managers with employee relations issues as well as coaching managers on corrective action process and performance management.

Complete requests for employment and salary history from various sources adhering to confidentiality policy.

Work with managers and payroll to assist in answering employee payroll questions.

Assist with internal and external reviews and audits by pulling personnel files and verifying files are accurate.

Attend required training/in-services to gain and maintain knowledge of regulations and requirements.

Perform other duties as assigned.

QUALIFICATIONS:

High school diploma or equivalent with at least 1 year experience in HR having knowledge of Human Resources concepts and law, practices, and procedures.
Excellent computer skills and proficient in Microsoft Office products.
Prior experience working with an HR database essential.
Must possess ability to analyze and research data and prepare reports as directed.
Must have the ability to work independently with good time management and good organizational skills.
Demonstrated ability to handle highly confidential information.
Must be able to travel to various worksite locations in Ohio.


PHYSICAL DEMANDS:

Direct Support Manager

FULL TIME POSITION: Direct Support Manager – Champaign County (Urbana, OH) at CRSI. 

BASIC FUNCTION:

Responsible for the overall management of the home with emphasis on providing a ‘homelike” environment to the individuals residing in the home. Based on each consumer’s Individual Plan, the Support Manager will coordinate implementation with consumers to help meet their various social, physical, and psychological needs. Ensure that CRSI operates under the current standards and regulations established by DODD Medicaid (Title XIX), and other authorities that may dictate standards of operation including programming, consumer safety, and staff training and employment requirements.

PRIMARY RESPONSIBILITIES

As a member of the interdisciplinary team, provide assistance in planning and implementation of the person centered services/outcomes for each consumer per the individual’s plan, communicate progress made by the consumers related to these services/outcomes, establish and maintain professional relationships with all members of the team, ensure the welfare and safety of each consumer at all times.
Coordinate and monitor the overall operation of the Residential Home, to include but not limited to staff supervision, on-call responsibilities after normal business hours, including nights and weekends, coordination of all activities, and maintenance of home. Schedule appropriate levels of staff supervision as required by consumers Individual Plan.
Ensure all required reports, documentation, and record keeping are thoroughly completed and maintained.
Ensure compliance with all trainings, in-services, and employment requirements.
Ensure agency policies and procedures are followed at all times and, when warranted, ensure timely delivery of disciplinary action.
Provide recommendations for hiring and developing employees.
Performs other duties as assigned.

QUALIFICATIONS:

Must be 18 years or older.
Must have a high school diploma or equivalent.
Must have strong communication, writing, time management, and organizational skills.
Must be able to use discretion and make independent decisions based on good judgment. Must possess good problem solving and conflict resolution skills.
Must be able to multitask.
Must have a valid driver’s license and vehicle insurance.
Must possess acceptable employment and background screenings.
